Job Description: This role is within our Patient Connect program. Optum Complex Care Management provides the Patient Connect Program for United Healthcare members in a short stay/transitional setting with focus on reduction in 30-day hospital readmissions, improved completeness and coding accuracy of diagnosis and medical record documentation, increased closure of STAR / HEDIS quality measures, improved completion rates of Advanced Care Planning Directives, and improved patient and family satisfaction and discharge and post discharge support. Job Responsibilities: Documentation, coding and gap closure Collaborate with the nursing staff and the patients' families Work with primary care physicians to provide the best care possible May be asked to help with our ISNP program which is providing primary care to patients in long term care settings Collaboration with case management team Job Requirements: 1+ years of hands on post grad experience within Long Term Care Nurse Practitioner national certification as ANP, FNP, or GNP required or the ability to obtain Current, unrestricted NP license in North Carolina or the ability to obtain Intermediate level of proficiency in PC based word processing including Microsoft Word and Outlook Experience in short term care / rehab Experience working in a nursing home or with seniors in other settings Ability to obtain DEA licensure / Prescriptive Authority post-hire in states where applicable Clinical background in adult, family or geriatric specialties Job Details: Company: United Healthcare Vacancy Type: Full Time Job Location: Winston-Salem, NC, US Application Deadline: N/A Apply Here getyourvacancy.com
